CS Studio Online Rental Form
This agreement confirms the rental of Constellation
Salon Studios (CS Studios) for the day(s) and rate specified and obligates
the undersigned to pay the amount of the rental noted below, including any
additional unforeseen expenses incurred during the dates of the rental.
Unforeseen expenses might include, but are not limited to; additional rental
days, overtime, equipment rental, photo and office supplies, phone/fax charges,
damages, clean-up fees, waiting time for sets, props,equipment, etc. WAIVER OF LIABILITY: Rental Terms: An Insurance certificate from your
insurer naming Constellation Salon Studios as additionally insured on the dates
of the rental must be sent before the rental dates.
For Payment,
you may use major credit card via PayPal (click on button below) or mail
a certified check. Fax or mail a copy of your Liability Insurance Certificate.
(You can check with Tom C. Pickard & Co., Inc. for insurance.) Payment for your rental must be received before
your reserved time begins. You may pay upon arrival to our studio in cash only.
Constellation Salon Studios rents
its facility, including limited
equipment, to its customers with the understanding that in no event shall Constellation
Salon Studios be held liable for direct, indirect, incidental, or consequential
damage, mishaps, accidents, injury or loss due to the use of this facility or
equipment used by its customers.
Renter hereby waives
and holds harmless Constellation Salon Studios, its owners, agents,
employees, affiliated independent contractors and management from any incident
or accident which may occur to or by persons either renting or associated with
the renting of Constellation Salon Studios on the dates noted below, or
any additional dates added later. Renter agrees to waive the right to seek
legal redress for mishaps, accidents, and loss while on our premises. Renter also agrees to have read, fully understand
and agree to be bound by the general rules and payment terms of Constellation
Salon Studios. You will be required to sign and date a copy of
this form before your rental begins.
